Grove City Medical Center has been doing a better job than most at preventing readmissions.A Cigna studyfound that telephonic outreach lowered readmission rate by 22%.UW Hospital has bought the Best Western InnTowner Hotel to house patients from around the region before procedures, during extended treatments and for post-discharge lab tests and education.Proteus has developed a technology based on an ingestible sensor that can track how well patients are adhering to their medications.On January 22nd, Right at Home CEO Allen Hager talked with Fox Business News about the Right at Home program Right Transitions.

Illinois Hospitals Take Measures to Reduce Readmissions

More than two dozen Illinois hospitals face Medicare cuts totaling as much as $19 million next year under the new federal health care law because too many of their patients are being readmitted for heart attacks and other ailments. St. John's Hospital and Schnuck Markets have teamed up to reduce readmissions by improving post discharge medication adherence. NorthShore University Health Systems has applied data analytics tools to focus on reducing its readmission rate from 21% to 17% over a period of six months.
