What Are Care Transitions?
Care transitions refer to a medical patient moving from one healthcare provider or healthcare setting to another. Seniors with illnesses, for example, may visit a primary care physician and then be admitted to a hospital and receive treatment from a hospital physician and nursing staff.
Illinois Hospitals Take Measures to Reduce Readmissions
More than two dozen Illinois hospitals face Medicare cuts totaling as much as $19 million next year under the new federal health care law because too many of their patients are being readmitted for heart attacks and other ailments. St. John's Hospital and Schnuck Markets have teamed up to reduce readmissions by improving post discharge medication adherence. NorthShore University Health Systems has applied data analytics tools to focus on reducing its readmission rate from 21% to 17% over a period of six months.
