Right at Home of Louisville, Kentucky and South Central Indiana and our community partners, to include SeniorCare Experts, GuardiaCare,Syntricity Rehab, Senior Pharmacy Solutions and Kent School of Social Work will not only help you reduce hospital readmissions for your loved one, in many cases we can help to ensure that they're not admitted in the first place. We have developed a model is extremely comprehensive and will provide the for best possible continuum of care for those who utilize RightTransitions in our respective communities.
Our model includes not only non-medical homecare, companionship, respite care, alzheimer's care, dementia care and personal care - it includes geriatric care managment, access to social workers, meal delivery, medication planners, evaluation of medications by a pharmacist, fiduciary services, navigator services, guardianship (if needed), plus much more.
