Whether you’re being discharged from a healthcare facility to your home or need an extra set of hands as you move into a new residence, Right at Home’s specially trained caregivers can provide both homemaker/companion and personal care assistance tailored to your needs and preferences. Nursing oversight and visit documentation is included in all packages.
Packages start at just $69 and can include any or all of the following services:
Personal Care
- medication reminders
- mobility/ambulation assistance
- personal hygiene/grooming assistance
- shower/bathing - stand-by or full assist
- client physical status monitor/documentation
- unique situations (special diets, food intake, blood pressure monitoring and documentation)
Companion Care
- meal planning
- cooking
- escort to dining room and group activities
- laundry/homemaking/unpacking
- transportation for medical appointments
- errands/grocery and prescription pick-up
- unique/personalized companion activities
Welcome Home /Move In- Package # 1
- Transportation to home
- Unpacking, re-acclimation to home environment
- Three (3) hours one-on-one service
- Phone follow-up by Nurse
Short Term “Helping Hands” - Package # 2
- Nurse visit and short term care plan created
- Six (6) hours one-on-one service (minimum 2 hour increments)
- Nurse follow-up phone call for final assessment and exit recommendations.
Short Term “Helping Hands” + Transportation - Package # 3
- Nurse visit and short term care plan created
- Eight (8) hours one-on-one service, including transportation (minimum 2 hour increments)
- Nurse follow-up phone call for final assessment and exit recommendations.
