Hospital to Home Services in Alameda County

After a stay at the hospital or other care facility, there’s nothing like returning to the comfort of home. A discharge from the hospital can seem like a welcome end to an injury or illness. But the transition to living at home can be daunting. It’s difficult to manage appointments, medications, and the routines of daily living such as grocery shopping and housework when all you want to do is rest and recuperate. That’s why we developed our own hospital to home program called RightTransitions® .

Through our RightTransitions hospital to home care program, Right at Home Alameda County can help seniors and adults with disabilities get home safely and prevent further hospitalizations with regular visits by our trained caregivers. We’re here to make living at home easier, so you can save your energy for recovery.

"Right at Home was the most helpful company I have ever worked with. My parents would not have been able to stay in their home in the last three years without the excellent care they received from Wendy and Anna. Even help that came in for one or two days was excellent and very caring. "

"The most important thing is they make sure I take my medications, they help me with drying and adding lotion after my shower, and they put on my socks and shoes. They purchase some of my needs with grocery shopping, and they make sure I have enough pads and supplies because I am incontinent. I use them also to carry things up to the road for the garbage man. They are very caring, they keep in touch, and they are very aware of the scheduling problems that some folks have."
