A More Supportive Journey for Those Who Are Navigating Dementia

Right at Home clients and families in the Englewood area affected by dementia can be evaluated for no-cost support services from PocketRN, a leading provider of virtual nursing care. PocketRN was recently selected as a participant in the Guiding an Improved Dementia Experience (GUIDE) Model by the Centers for Medicare & Medicaid Services (CMS).

As a GUIDE partner, PocketRN’s "Nurse for Life" model will enable Right at Home clients in Englewood with dementia—along with their family caregivers—to be specialty matched to nurses who will become trusted companions in navigating care and changes.

Right at Home clients who enroll with PocketRN receive:

  • A dedicated, dementia-trained nurse for ongoing support via phone/video check-ins
  • 24/7 virtual access to a trusted care team
  • Respite hours (paid home care hours) for family caregivers of those who qualify
  • PocketRN care coordination with doctors, specialists, and medical suppliers
  • Medication management
  • Family support, education, and 1:1 coaching for caregivers
  • Management of chronic conditions (e.g., diabetes, heart disease, cancer)
  • Referrals to Valparaiso community resources and additional support services
  • And more!

PocketRN nurses also encourage openness to better understand individualized challenges; serve as continuous sources of education, coaching, monitoring, and emotional support; and are available as a 24/7 lifeline.

Under the GUIDE Model, PocketRN will be available to Right at Home clients in Valparaiso at no cost to eligible beneficiaries, with $0 copays.

Additionally, annual respite relief hours are available for family caregivers of those who qualify.
