Care Coordinator - Issaquah

The Care Coordinator (CC) in the Right at Home Issaquah office is responsible for the day-to-day scheduling and coordination of caregivers, taking client inquiries, and resolving issues that may arise between a client, their care team, and Right at Home care staff. The CC works as part of a multi-disciplinary team to ensure that all client’s needs are expertly met and provided in a timely manner. The CC’s role is to demonstrate the company’s culture of compassion, accountability and results by displaying the highest standards of integrity, superb follow through, consistency, exceptional customer service and client satisfaction. The CC is responsible for liaison work with community and professional partners as well as directing client centered care and building and maintaining client files.

Essential Functions:

  • Schedule client cases with appropriate caregiving staff, demonstrate empathy and understanding of the needs of the client and needs/abilities of the caregiving staff; take all steps necessary to produce a complete and effective schedule
  • Communicate responsibilities to caregiving staff, track attendance, document appropriately and monitor performance against Plan of Care
  • Verification of caregiver timesheets and client bill rates, monitoring telephony system, other payroll responsibilities when necessary, as well as other office coordination duties
  • Open communication with Sales and Marketing with insight on key referral sources
  • Act as liaison amongst clients, their families, community partners, care staff, and office staff to ensure highly effective patient centered care outcomes
  • Promote effective Home Care services in compliance with Agency philosophy, policies and procedures, and in accordance with State licensing laws and regulations
  • Participate in referral/intake process for clients needing Home Care services, as well as quality improvement activities and evaluations of the Agency
  • Participate in after hours, weekends and holidays on-call duties outside of normal business hours (rotating schedules)
  • Work closely with office staff to promote communication and assure appropriate client care documentation
  • Preparation of essential client and/or referral packets

Qualifications:

  • Ability to perform non-medical client assessments including documenting medical history, physical health, mental health, and social and environmental needs
  • 2-3 years of scheduling experience is preferred • Knowledgeable in Microsoft Office Suite, experience with ClearCare preferred
  • Demonstrate self-motivation, self-direction, organizational skills, flexibility and ability to enthusiastically face a high level of stress and constantly changing priorities
  • Must maintain an active caregiving license in the State of Washington (i.e. HCA, NAC, CNA). If not licensed must be willing to complete HCA course within 90 days of hire and obtain HCA license within 120 days of hire
  • Excellent written and verbal communication skills with clients, families, caregivers and administrative staff
  • Driver’s License and insurance as well as access to reliable automobile
  • Must pass background check & initial drug screen
  • Must provide proof of successful negative TB Screening, and must provide proof of Hepatitis B Vaccination
  • Must complete 12 hours of in-services annually
  • Must be 18 years of Age or older with US Citizenship and or equivalent employment authorization
  • Read, write and understand English language
  • Must be available for rotating on-call duty outside of normal business hours
