Personal Care Assistant/CNA (Milwaukee Waukesha, WI)

Provides various personal care and related paraprofessional services in accordance with an established Plan of Care.  Provides for the personal needs and comfort of clients in their homes.

Essential Functions

  • Adheres to Agency policies and procedures
  • Performs work under the supervision of the Client Care Coordinator.
  • Performs personal activities contained in a detailed Plan of Care by the Client Care Coordinator which includes, but is not limited to, assisting the patient with activities of daily living, such as:
  1. Personal Hygiene
    1. Helping client with toileting (or in using bedpan/urinal)
    2. Bathing
    3. Care of mouth, skin, and hair
  2. Ambulation
  3. Eating
  4. Dressing
  5. Shaving (electric razor) *DON’T SHAVE IF DIABETIC*
  6. Meal Preparation

Performs other duties as specified in the client’s plan of care.  These include but not limited to:

  • Assisting with the change of ostomy appliances
  • Reinforcement of dressing (non-sterile)
  • Assisting with the use of devices for aid to daily living (cane, walker, wheelchair or lift device)
  • Assisting with self administered simple urine/blood tests for sugar acetone or albumin, but may not interpret results
  • Assisting with prescribed range of motion exercises
  • Measuring and preparing special diets
  • Measuring intake and output
  • Taking vital signs (temperature, pulse, respirations, and blood pressure)
  • Application of an external (condom-type) catheter
  • Assist with application of arm sling, arm splint, or leg splint

The Caregiver/PCW/CNA will perform light housekeeping tasks that will facilitate the client’s self-care at home.  These tasks include, but are not limited to:

  • Vacuuming
  • Dusting
  • Sweeping / mopping floors
  • Washing dishes / running dishwasher
  • Laundry (Wash/Dry/Fold/Iron)
  • Cleaning/Disinfecting bathrooms
  • Changing linens

The Caregiver/PCW/CNA will also provide transportation for client’s in any of the following situations with prior office approval:

  • Doctor Appointments
  • Grocery Store
  • Pharmacy or picking up an errand
  • Scheduled Activity

The Caregiver/PCW/CNA is responsible for documentation of services provided, using the Daily Care Record. Any client condition changes or questions will need to be addressed to the Right at Home Office immediately. 

Additional (non-essential) Desired Functions

  • Performs other duties as directed by supervisor

The Caregiver/PCW/CNA will NOT perform any of the following:

  • Sterile dressing change
  • Irrigation of body cavities such as enemas, colostomy or wound
  • Gastric lavage or gavage
  • Catheterization
  • Administration of oral medications, eye drops, or medicated skin ointments
  • Administration of parenteral or IV medications
  • Application of heat by any method
  • Naso-gastric or gastrostomy tube feedings or care
  • Tracheotomy tube care
  • Suctioning of the nose, mouth, or tracheotomy
  • Nail care on a patient with diabetes or compromised circulation
  • Any personal health service that has not been included on the paraprofessional task sheet
  • Perform heavy-duty household cleaning such as washing walls, waxing floors, washing windows, etc.
  • Household repairs or maintenance
  • Yard care or maintenance

Education, Experience, Knowledge, Skills, Abilities and Availability

  • High School Graduate or G.E.D. Certificate preferred
  • Six months experience providing personal cares in homes or facilities
  • and/or successful completion of the State approved Nurse Aide or
  • and/or successful completion of the Personal Care Assistant course
  • and/or successful completion of CBRF
