Right at Home Provides Dementia Care to New Clients Through GUIDE Model

The Centers for Medicare & Medicaid Services’ Guiding an Improved Dementia Experience (GUIDE) Model has been in existence for nearly one year. Right at Home has partnered with PocketRN, a virtual nursing care platform, to provide care for dementia patients through the GUIDE Model. McKnight’s Home Care reported on Right at Home and PocketRN’s early success with improving dementia beneficiaries’ quality of life through the GUIDE Model.
