Care Manager Assistant

A Care Manager Assistant works under the supervision of a Care Manager. The position coordinates client communications, meetings, caregiver rotations, training and evaluations, assists with office operations, and provide client care if/when caregivers' schedules present a need.

Caregiver

A Caregiver provides for the maintenance of a safe and clean environment. Performs various housekeeping tasks, meal preparation and caregiving activities for clients in their residences.

Certified Nurse Assistant (CNA)

A CNA provides various personal care and related paraprofessional services in accordance with an established plan of care. Provides for the personal needs and comfort of patients in their homes.

Homemaker/Companion

A Homemaker/Companion provides for the maintenance of a safe and clean environment. Performs various housekeeping tasks, meal preparation and companionship activities for clients in their residences.

In-Home Health Aide

An In-Home Health Aide performs various housekeeping tasks, meal preparation and caregiving activities for clients in their residences. This position essentially provides for the maintenance of a safe and clean environment for clients.

PRN LVN or RN

The PRN LVN and RN private skilled role requires someone who can work independently, demonstrate client and caregiver management skills, and have a servant's heart. This includes responding to customer requests in an efficient and professional manner, including clients, families of clients, and referral sources.
