When you contact Right at Home for assistance at discharge, we are responsive to to your needs day or night, and can even start in-home care the same day when necessary. Often meeting families in the hospital or rehabilitation center to discuss resources, options, and prepare home care before discharge, we collaborate with other medical professionals so needs are met before returning home.

After discharge, we offer a free in-home assessment to further discuss needs of your loved one as well as provide recommendations for the home to ensure safety. The RightTransitions program also helps you get a head-start on daily living activities such as house safety, housekeeping, laundry, meal prep, safe transfers, med reminders. When tapped to help at discharge Right at Home allows you to focus on recovery while our team of specialized caregivers take care of the rest!

Custom Transition Services to Meet Your Needs

Right at Home OKC offers convenient Transition Packages that can be customized to fit your needs. Our packages range from three hours to 24/7 care and typically include:

Supervised personal care assistants and companions
Trained, screened, bonded and insured caregivers
Bathing, dressing and toileting
Shopping for groceries and running errands within our scope of services*
Free assessment to determine additional resources or services needed
Meal preparation and light housekeeping
Medication reminders
Assistance to appointments within our scope of services
Dependable transportation along with transfer assistance to and from most destinations*

Right at Home also partners with many local home services companies. These companies can be beneficial by assisting in recovery and providing support with home safety equipment, medical home-health equipment, physical therapy, and personal training.

Resources to Make Recovery at Home Easier

Discharge assistance. We get you home from the hospital, settled in and comfortable, so you can get started healing.

Communication. We communicate with your healthcare providers on a regular basis to ensure your recovery is progressing as expected and notify them of any changes in your condition to head off any potential complications.

Medication reminders. Managing new medications is an important part of your recovery. Our caregivers make sure you take the medication you need, when you need it.

Transportation. If getting to the pharmacy, grocery store or appointments is a challenge, your recovery could be at risk. We provide transportation to get you where you need to be, so you can stay where you want to be: right at home.

Coordinate appointments. After a hospitalization, there can be a flurry of new appointments. Whether you need follow-up appointments with your general practitioner, a new specialist or a physical therapist, count on us to help you keep it all organized.

Meal preparation. Nutritious meals are vital to a successful recovery. If you have been prescribed a new diet, let us help. Our caregivers prepare nutritious meals for any diet — even special diets for conditions such as diabetes or congestive heart failure.

Housework. Your focus is on your recovery, not housework. Our caregivers take care of typical daily chores, such as vacuuming, washing dishes and cleaning the bathroom.

Safety. You may be a little unsteady on your feet as you regain your strength. We can help prevent falls and assist you with movement and walking.

Hygiene. Bathing, dressing and toileting can be difficult. Let our caregivers help you maintain your independence while recovering.

My dad had been resistant to home companions in the past so I was very nervous how he would react to the Right at Home caregivers. They immediately put my worries to rest. They were very gentle and nonthreatening and were able to get my dad up in the morning and take him to his day program even when he didn't want to go.
