Hospital to Home

After a stay at the hospital or other care facility, there’s nothing like returning to the comfort of home. A discharge from the hospital can seem like a welcome end to an injury or illness. But the transition to living at home can be daunting. It’s difficult to manage appointments, medications, and the routines of daily living such as grocery shopping and housework when all you want to do is rest and recuperate. That’s why we developed our RightTransitions® program.

Right at Home Maricopa County Southwest can help seniors and adults with disabilities get home safely and prevent further hospitalizations with regular visits by our trained caregivers. We’re here to make living at home easier, so you can save your energy for recovery.

"Louis is 94. Right at Home was selected through the VA. All the caregivers they have provided have been very professional, friendly, and warm. They have helped me by staying with dad, it gives me peace of mind knowing he is not alone. The caregivers spend time and engage with him. They play cards and watch TV."

"I chose them because it was assigned by social workers. I like everything they do for my mother. They have helped my mother's behavior improve a lot. They are very patient with my mother when it comes to helping her with her personal hygiene, cleaning the house, and cooking her meals. Every time we have a question, they answer right away. They both speak Spanish, which makes it easy."
