Resources for Adults and Seniors with disabilities

CHAMPSS - Choosing Healthy Appetizing Meal Plan Solutions for Seniors

The San Francisco Department of Disability and Aging Services (DAS), in cooperation with Self-Help for the Elderly, offer a dining opportunity called CHAMPSS, a program for adults 60 or older and their spouse/partners regardless of age. The program provides seniors with a dietitian-approved menu, giving them the ability to sit down and enjoy a complete meal together.

Centre for Independence of Individuals with Disabilities

CID is an Independent Living Center, one of 28 throughout the state. They serve people with disabilities within San Mateo County. They are a small, private, nonprofit agency, and receive some of their funding through grants and donations. There are other ILCs covering various areas of the state. CID can provide the phone numbers and addresses of the other centers if you ask for them. They help individuals find solutions to their own problem, leading to their independence.

Self-Help for the Elderly

Founded in 1966, Self-Help for the Elderly is a community-based organization committed to promoting the independence, dignity, and self-worth of seniors. Each year the agency serves over 40,000 seniors in San Francisco, San Mateo, Santa Clara and Alameda Counties by providing a comprehensive range of multicultural and multilingual services to empower seniors to help themselves and others.

Avenidas: Re-Inventing Aging in the mid-Peninsula since 1969

Avenidas seeks to create a community that supports and celebrates older adults. They provide a wide range of programs, information, and services that enable people to stay active, maintain their independence, help their aging parents, or care for a spouse. Anything someone might need, from a ride to the doctor to a flu shot, Avenidas can help. Their innovative programs and compassionate staff have enriched the lives of more than 7,500 mature adults and their families each year.

Fall Prevention Coalition of San Mateo County

The Fall Prevention Coalition of San Mateo County was developed in March 2003 and brings together a coalition of more than 20 different community provider agencies, hospitals, nonprofit organizations, senior centers and private service providers, all serving older adults in San Mateo County.

 

In San Mateo County, falls account for 80% of accidental injury deaths in individuals over the age of 85, and 20% in ages 75 to 84.

 

The mission of the Fall Prevention Coalition of San Mateo County is to prevent falls among older adults in our community through collaboration, education, resource development and advocacy.

Alzheimer's Association Walk to End Alzheimer's®

Held annually in more than 600 communities nationwide, Walk to End Alzheimer's is the world's largest event to raise awareness and funds for Alzheimer's care, support and research.

 

While there is no fee to register for Walk, all participants are encouraged to raise critical funds that allow the Alzheimer’s Association to provide 24/7 care and support and advance research toward methods of prevention, treatment and, ultimately, a cure.
