Home Health Coordinator

Right at Home Seattle-Eastside is looking for a full-time Home Health Coordinator at our North Seattle office.

The Client Care Coordinator is responsible for the daily management of clients receiving in-home skilled nursing and/or nurse delegated care. The ideal candidate has experience in home health or in-home caregiving.

Office hours for the Client Care Coordinator are Monday – Friday, 8:30am-5pm (the office closes from 12:15-12:45 for lunch).


  • Managing staffing levels for our nursing clients
  • Coordinating nursing assessments and care plans
  • Manages Quality Assurance requirements and conducts monthly supervisory visits
  • Being the primary interface between field staff and office staff
  • Communicate with pharmacies and third-party care coordinators
  • Providing excellent customer service
  • Participate in our rotating on-call schedule (on-call duty is in the evenings and on weekends for a one-week period every 2-3 months)

Ideal Qualifications:

Experience in medical records - However, candidates with other clinical experience and backgrounds in in-home care services will be considered

Experience with scheduling in a health care field

Clinical experience as a Nurse Delegated CNA (Certified Nursing Assistant) or an active LPN (Licensed Practical Nurse) license with WA Department of Health

Strong organizational skills and ability to manage medical paperwork and files

Excellent written and verbal communication in English

Computer literacy with Microsoft Office products and the ability to write 35-45 wpm

A valid Washington State Driver’s License, current Auto Insurance, and a reliable vehicle

Ability to commute to our Seattle, WA office
