- Discharge Care — This package is designed to help in transitioning clients from medical facilities back to their homes, assisting with many of the needs of clients so they can get settled and comfortable. The first 24 to 48 hours after a hospital or skilled rehabilitation center stay are sometimes overwhelming. Our 'Discharge Package' is designed to help reduce the risk of re-hospitalization.
- Extended Discharge Care — Providing all the peace of mind of the 'Discharge Package.' this 'extended' version includes additional hours of support in the days following the return home. Sometimes it may take more than simply getting re-acquainted with being home the day of discharge. Some clients need a little extra care over time to start getting back to their routines of daily living. That's where the flexible hours and days of the 'Extended Discharge Package' can be personalized to best meet a client's needs.
- Respite Comfort Care — Caring for your parent, sibling, spouse, or loved one is no small feat. You know this, and so do we. You’ve sacrificed to serve in your role as caregiver for the one you love, and that’s why you deserve time to rest and revive your spirits through respite care. Our 'Respite Care Package' gives you an option to take that much deserved and needed break to re-charge and take care of yourself.
- Hospice Comfort Care — Hospice care is emotionally and physically draining for caregivers and loved ones. The 'Hospice Comfort Care Package' is specifically designed to give you a break, giving you time to recover from fatigue. You can't take care of others if you don't take care of yourself. Right at Home Sherman's specifically hospice trained caregivers can offer you the break you need.
