As you are readjusting to life after a hospitalization or outpatient procedure, caregivers can offer physical assistance getting out of bed and moving around the house.
1 - Discharge Plan
Cost: $99 flat fee
Service Length: Up to six hours of service within a ten mile radius.
Includes the following services:
- A safe ride home from the hospital or rehab facility
- Picking up medications and groceries
- Doing light housekeeping and meal preparation
- Getting client ready for bed
- Bathing / dressing assistance if needed
2 - RN/Care Coordination
Cost: $65 /visit
Service Length: Up to one hour per visit.
Includes the following services:
- Reviewing discharge reports from hospital or rehab facility
- Adjusting care plan
- Monitoring changes in condition
- Reporting back to clinicians in case of red flags
- Coordinating communication between providers
- Setting up medications.
3- Companion / Sitter
Cost: $19.50/hr
Service Length: visits lasting four or more hours.
Includes the following services:
- Monitoring changes in condition and reporting back to clinician and family
- Checking food intake and hydration
- Overseeing self-care and medication
- Preparing nutritious meal
- Light housekeeping and organizing
- Preventing wandering.
4- CNA / Personal Care
Cost: $22/hr
Service Length: visits lasting four or more hours.
Includes the following services:
- Assisting to/from toilet and incontinence care
- Assisting with transfers (with or without supporting gear such as gait belts and Hoyer lifts)
- Safely assisting into bathtub or shower
- Bathing the client
- Monitoring mobility to prevent falls
- Ostomy care (under nurse supervision)
