Right at Home Incorporates Change-in-Condition Services to Provide Additional Peace of Mind
Right at Home has implemented a unique program designed to improve our clients’ health outcomes called Change-In-Condition Care. It’s a proactive way to prevent conditions from worsening, and it gives families the peace of mind of knowing their loved one is not only being cared for, but monitored and observed for any declining health behaviors. To us, Change-in-Condition Care is an important part of keeping our clients safe and healthy in their own homes.
Prevents Readmissions and Hospitalizations
Change-in-Condition Care can avert preventable hospitalizations and re-admissions, particularly for recovering, high-risk seniors.
What is Change-in-Condition Care?
Right at Home’s Change-in-Condition Care program starts with training. Our caregivers and office staff are trained to observe, report, and act on any changes in the client's condition, particularly in the following five change-in-condition categories:
- Eating and Drinking
- Skin Condition/Swelling
- Overall Differences
How It's Reported
At the conclusion of every shift, Right at Home’s proprietary system requires our caregivers to affirmatively report whether their client has had any changes in condition. If so, the caregiver describes the change in condition, which is then automatically forwarded to our office staff and/or if there is an RN on staff. At that point, our office team communicates the change in condition to the client’s care stakeholders, including:
- Primary Care Physician
- Social Worker / Discharge Planner / Case Manager
- Home Health Care / Hospice
We then work with the family and relevant medical professionals to re-assess the client’s needs and revise the client’s customized care plan as appropriate. Finally, the client’s caregivers are instructed on how to optimally perform their caregiving services under the revised care plan. Only then will our office staff “close out” the change in condition protocol for the client.
